Your covered team members enjoy one point of contact for all their medical needs. Our staff is a phone call or an email away, and ready to:

  • Call in a prescription refill request to your pharmacy
  • Schedule an appointment with other appropriate doctors and/or services (imaging, physical therapy, specialist, etc.)
  • Reschedule appointments when conflicts arise
  • Assist with healthcare needs while traveling
  • Attend appointments when appropriate or requested

Coordination of Care

Should follow-up diagnostics or treatment be recommended, we coordinate each client’s care to ensure a seam-less experience. This may include:

  • Select a specialist that is an ideal match for the client’s condition or needs.
  • Schedule an appointment with the specialist and communicate with them prior to the appointment.
  • Follow up with the client after the visit to review recommendations and facilitate follow-up care.
  • Consult with other specialists to make sure our client’s health action plan is current, appropriate and evidence-based.
  • Regularly return to the health action plan to ensure progress is being made.
  • Continue to discuss other conventional or non-conventional testing to further benefit the client’s healthcare needs.
