- Call in a prescription refill request to your pharmacy
- Schedule an appointment with other appropriate doctors and/or services (imaging, physical therapy, specialist, etc.)
- Reschedule appointments when conflicts arise
- Assist with healthcare needs while traveling
- Attend appointments when appropriate or requested
Coordination of Care
Should follow-up diagnostics or treatment be recommended, we coordinate each client’s care to ensure a seam-less experience. This may include:
- Select a specialist that is an ideal match for the client’s condition or needs.
- Schedule an appointment with the specialist and communicate with them prior to the appointment.
- Follow up with the client after the visit to review recommendations and facilitate follow-up care.
- Consult with other specialists to make sure our client’s health action plan is current, appropriate and evidence-based.
- Regularly return to the health action plan to ensure progress is being made.
- Continue to discuss other conventional or non-conventional testing to further benefit the client’s healthcare needs.
